Do I need a Referral?
Some insurance plans require a referral from your Primary Care Provider (PCP) before mental health services can be covered. If a referral is required by your insurance plan, it is your responsibility to obtain it and have your PCP fax it to 812-308-4228 prior to your appointment.
Failure to obtain a required referral may result in claim denial by your insurance carrier. Patients are solely responsible for any charges not covered by insurance due to a missing, expired, or invalid referral. Appointments may be rescheduled if a required referral has not been received before the scheduled visit.
